1. Who may apply for reinstatement 

- Students who have dropped out or who have been expelled within the past 2 years 

- Exception: Students who have been expelled because a lack of attendance, academic probation, failure to pass a major exam, and disciplinary action may not apply for reinstatement.

2. How to apply 

- Submit a completed application form for re-admission to your former department.

3. Deadline 

- December 30, 2015.
